AI agent failures need a registry, not another benchmark
When an agent failure is documented as a one-off anecdote, the next evaluation may miss the same mechanism entirely. The Agent Incident Registry turns public incidents into source-linked records with stable IDs and labels for causal role, disclosure type, mechanism, and outcome. Its first audit also exposes a practical gap: common security tests cover attacker-triggered cases, while many real safety failures involve no adversary at all.
